We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will be responsible for evaluating and treating patients with various physical ailments, developing individualized care plans, and providing exceptional patient care in a variety of settings, including inpatient, outpatient, and home health environments. This role requires a strong understanding of anatomy and physiology, as well as the ability to work collaboratively with other healthcare professionals to ensure optimal patient outcomes.
*Duties*
* Conduct thorough assessments of patients' physical abilities and limitations.
* Develop and implement personalized treatment plans based on individual patient needs.
* Provide hands-on physiotherapy treatments, including trigger point therapy and orthopedic rehabilitation.
* Monitor patient progress and adjust care plans as necessary to achieve desired outcomes.
* Collaborate with interdisciplinary teams to coordinate discharge planning and ensure continuity of care.
* Educate patients and their families about treatment options, exercises, and self-care strategies.
* Maintain accurate documentation of patient evaluations, treatment sessions, and progress notes using EMR systems.
* Stay updated on the latest advancements in physiotherapy practices and medical terminology.
*Skills*
* Strong knowledge of anatomy and physiology relevant to physical therapy practices.
* Proficiency in discharge planning and developing comprehensive care plans for diverse patient populations.
* Experience in acute care settings, geriatrics, outpatient clinics, and home & community care environments.
* Familiarity with medical terminology and EMR systems for effective documentation.
* Ability to provide compassionate patient care while fostering a supportive therapeutic environment.
* Excellent communication skills to effectively interact with patients, families, and healthcare team members.

Pay: From $90.00 per hour
