*Position Summary*
Oak Park Estates is seeking a compassionate HealthCare Coordinator to lead the clinical operations of our Memory Care Assisted Living community.
This leader is responsible for ensuring the highest standards of resident-centered care, overseeing nursing and guiding our team of caregivers, while maintaining regulatory compliance, and supporting a positive, engaged, and stable team culture.
*Key Responsibilities Include*:
Perform or oversee pre-admission assessments of residents’ functional, cognitive, and health status.
Develop, implement, and regularly update individualized service plans for each resident.
Monitor residents for significant changes in condition and ensure timely assessments, interventions, and communication with families and providers.
Ensure all assessments, care plans, and documentation are current and compliant with state regulations.
Oversee all aspects of clinical care and nursing procedures within the Memory Care Assisted Living community.
Support and maintain a safe, homelike environment that supports residents’ dignity, purpose, and well-being.
*Resident & Family-Centered Care*
Support a philosophy that puts residents first by providing care tailored to each person's preferences, background, and abilities with empathy and understanding.
Serve as a primary point of contact for residents and families regarding clinical care, changes in condition, and care planning.
Communicate proactively and transparently with families and providers to build trust, reduce anxiety, and support partnership in care.
Support and collaborate on activity and wellness programs that promote engagement, purpose, and meaningful daily life for residents with dementia.
*Team Leadership & Development*
Partner in recruiting, hiring, training, mentoring, and retaining caregivers, fostering a positive, high-performing work culture.
Provide ongoing education and support in care, and providing services based on best practices, and quality resident engagement.
Lead by example in promoting teamwork, respect, healthy boundaries, and clear communication.
Conduct performance feedback and coaching to support growth, accountability, and job satisfaction.
Participate in the on-call rotation to support clinical needs after hours.
*Medication & Health Services Oversight*
Oversee and lead the medication administration process, ensuring accuracy and adherence to physicians’ orders.
Coordinate health services with external providers and physicians, ensuring continuity of care and timely interventions.
Build and maintain strong relationships with local healthcare providers and vendors to support resident needs.
*Regulatory Compliance & Quality Improvement*
Ensure compliance with all state regulations, company policies, and standards of excellence in care.
Participate in quality assurance and performance improvement efforts, reviewing incidents, trends, and outcomes.
Maintain accurate, timely records and documentation to support regulatory readiness and quality of care.
*Qualifications*
Registered Nurse (RN) or BSN
Current state nursing license in good standing.
1–2 years of leadership or supervisory experience (senior living, long-term care, assisted living, or similar setting).
CPR and First Aid certifications (or willingness to obtain upon hire).
Strong skills in communication, problem-solving, decision-making, and time management.
Ability to prioritize and manage multiple responsibilities in a dynamic environment.
Comfort with basic technology and electronic health record (EHR) systems (or ability to learn quickly).
A genuine passion for serving seniors and commitment to our mission and values.

Job Type: Full-time
Pay: $38.00 - $41.00 per hour
Expected hours: No less than 40 per week
