*Currently recruiting for the following areas/counties:*
* Santa Clara County
* Alameda County
* Contra Costa County
*Qualifications and Skills:*
* Valid CA OT License
* Valid driver's license
* Physical Exam
* CPR certification
* PPD or Chest X-RAY
* At least One Year Experience in an Acute Setting
* Excellent customer service and patient focus
* Great clinical assessment skills
*Responsibilities:*
* Performs Occupation Therapy assessments of patients in their homes.
* Provide therapeutic treatments and evaluation of equipment needs.
* Prepares clinical documentation including assessments, visit and progress notes, and coordinates services as directed by the Case Manager and/or Patient Care Coordinator.
* Submits all documentation within timelines determined by the agency. Evaluate outcomes of care.
* Provides patient and family education
We are also looking for the following:
Home Health Physical Therapy Assistant (PTA), Registered Nurse (RN), LVN, Occupational Therapy, Certified Occupational Therapy Assistant (COTA), Speech Therapy, Medical Social Worker (MSW), Home Health Aide (HHA)
Job Types: PRN, Contract, Per diem
Pay: From $90.00 per hour
Expected hours: 2 – 40 per week
