The staff Physical Therapist assumes responsibility and accountability for a designated group of patients and provides Physical therapy evaluations, modalities and treatments in accordance with established hospital and departmental policies and procedures, maintaining the highest degree of quality patient care. Accurately assesses patient’s needs; identifies and initiates appropriate Physical therapy interventions, provides patient and family education and works cooperatively with other patient team personnel in maintaining standards for professional Physical therapy practice and multidisciplinary teamwork. Participates in performance improvement and program development activities.

*Minimum Qualifications:*
* Ability to project a Professional Image.
* Knowledge of regulatory standards and compliance requirements.
* Working knowledge and ability to apply professional standards of practice in job situations.
* Strong organizational and analytical skills.
* Working knowledge of computer and software applications used in job functions (Order entry, data input)
* Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a health care related setting are ineligible for employment in this position.
* Freedom from illegal use of drugs.
* Must have sensory skills to differentiate clinical signs and symptoms and the emotional stamina to deal with a variety of stressful situations including emergency response to patient incident.
* Freedom from use of and effects of use of drugs and alcohol in the workplace.
*Education and/or Experience:*
* Must hold a bachelor or master of physical therapy from an accredited college or university.
* Must hold a current Louisiana State License as a Physical Therapist and must maintain license renewal in accordance with the applicable State Licensing Board.
*Certificates, Licenses, Registrations:*
* Continuing CEU’s as required per individual licensure.
*LANGUAGE SKILLS*:
English is the primary language of the facility. Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations. Ability to chart patient information correctly. Ability to effectively present information and respond to questions from supervisors, patients and families, physicians and the public.
*MATHEMATICAL SKILLS*
* Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to compute rates, ratio, and percent.
* Must have sensory skills to differentiate clinical signs and symptoms and emotional stamina to deal with a variety of stressful situations including emergency response to patient incidents.
*REASONING ABILITY:*
Ability to apply common sense understanding to carry out instruction furnished in written, oral and/or diagram form. Ability to define and solve problems, collect data, establish facts and draw valid conclusions.
*ESSENTIAL FUNCTIONS AND RESPONSIBILITIES*:
To perform this job successfully, an individual must be able to perform each key function satisfactorily based on five overall skills categories: assessment, planning, intervention, evaluation and teaching/coaching. Each category will be assessed on performance measurements of appropriateness, efficiency, effectiveness and timeliness. Reasonable accommodations may be made to enable individuals with disabilities to perform the key functions.
*Additional Responsibilities:*
* Verifies physician’s orders for treatment prior to performing evaluation and well as conducts full chart review. Verifies critical/abnormal lab values with nursing staff prior to patient treatment.
* Evaluates each patient to establish, initiate and update an appropriate plan of care (POC) in collaboration with the patient and/or family in accordance with physician orders.
* Establishes specific, measurable, attainable, realistic and timely long and short term goals relative to the patient’s age and lifestyle.
* Works to develop, improve, or restore skills in activities of daily living, work or productive activities and play or leisure using and individualized plan for each patient.
* Uses intervention involving therapeutic use of meaningful and purposeful occupations, adaptation of environments, and promotion of health and wellness.
* Uses graded tasks and activities as prerequisites to engagement in occupations.
* Recommends design, fabrication, application or training in the use of assistive technology or orthotic/prosthetic devices, application of physical agent modalities and application of ergonomic principles to the performance of occupations.
* Works with the patient and/or family as well as the interdisciplinary team to establish an appropriate discharge plan.
* Initiates patient/family education when appropriate in a timely manner in accordance with physician orders.
* Provides treatment each patient on the 6th visits in accordance with APTA guidelines.
* Updates POC every 5-7th day of admission in accordance with IRF-PAI standards. Documents patient care encounters and charges in a legible, timely and efficient manner.
* Provides thorough, accurate and timely documentation providing information on continuity of progress, and reimbursement needs as well as skilling factors for discharge planning. Documentation also includes education of use of DME for justification purposes.
* Performs chart corrections within 72 hours.
* Works cooperatively with supervisors, physicians, therapy and nursing staff to contribute to the overall productivity of the team.
* Recognizes appropriate situations to utilize support from certified nursing assistants, and therapy aid to ensure patient and employee safety.
* Maintains a safe environment for patient care and maintains proper safety measures with treatment when able (i.e. gait belt, chair/bed alarm)
* Consistently meets patient care productivity standards as established for job classification.
* Collaborates effectively with other members of the health care team.
* Provides input at weekly interdisciplinary team meetings (staffing).
* Provides in-service education after attending clinical continuing education seminars as assigned by the Director of Therapy. \\
* Wears proper employee identification while working on premises.
* Performs modalities (i.e. ice, moist heat) when appropriate.
* Exhibits adaptability in assignments as it relates to caseload, scheduling, and staffing. Takes initiative to assist in other patient care areas and accepts responsibility to all patient care needs.
* Exhibits ethical and professional behavior and is courteous with patients, co-workers, family members, physicians and the general public at all times.
* Adheres to the professional standards of practice guidelines and code of ethics as set forth by the state board of physical therapy.
* Arrives before scheduled treatment time to conduct chart reviews.
* Notifies the therapy director of illness preventing him/her from attending work 2 hours prior to start time.
* Demonstrates active listening techniques; gains support through effective relationships; treats others with dignity and respect; seeks feedback; maintains reporting mechanisms.
*CONTINUING EDUCATION:*
Attends in-service and education programs; attends continuing education required for maintenance of professional certification or licensure.
*PHYSICAL DEMANDS AND WORK ENVIRONMENT*
The physical demands described here represent those that must be met by an employee to successfully perform the essential functions of this job. The noise level in the work environment is usually moderate.
*Work Hours:*
* Daily work hours – 8-hour shift with 30 minute deduction for lunch per day.
* Two 15-minute breaks are recommended.
* Staff responsible for filling out time off requests to use Vacation/Sick hours and turning into supervisors prior to end of pay period once approved.
* Overtime must be approved.
* May be required to work weekends on an intermittent basis as determined by the Director of Therapy.
* Holidays as assigned by Director of Therapy.
· Hours may be reduced due to low patient census
