*Current MDS Coordinator (RN) Openings:*
* Full-time (0.9-1.0 FTE), Day shift (~8:00 AM - 4:30 PM) \*Will include monthly/every 4th weekend rotation (Saturday & Sunday)
*Compensation: *
* *Full-time MDS Coordinators (RNs)* start at $37.68 (with no long term care experience) or $38.68 (with long term care experience), with MDS experience credit up to $40.18/hour.

*Primary MDS Coordinator (RN) Responsibilities*:
* Complete the Minimum Data Set (MDS) in an accurate and timely fashion upon admission, quarterly, annually, and when significant change has occurred.
* Complete comprehensive evaluations of all MDS-triggered condition utilizing the RAPs (Resident Assessment Protocols).
* Apple the RAI information to each residents' individualized care plan.
* Involve the interdisciplinary team in resident assessment, documentation of findings, analysis of information, and decision-making regarding care planning.
* Provide training and ongoing consultation to Program Managers and other floor staff in documentation for MDS/Case Mix.
* Monitor Quality Indicator Reports and provide ongoing education to direct care staff on quality indicator-related issues.
* Complete initial and annual Health Education/Psychiatric Skills Assessments and program recommendations for each resident.
* Develop the initial Index of Physical/Medical Concerns and to review and update at least quarterly.
* Complete the Urinary Incontinence Assessment and Care Plan when incontinence exists, to re-assess at least annually, and to review the plan of care quarterly.
* Review residents' Individual Program Plan, suggesting needed additions or changes to the Program Manager and to sign each IPP when complete.
* Participate in residents' initial and annual program review meetings and in quarterly review meetings when appropriate.
* Consult and collaborate with floor nurses about resident physical health issues and medical concerns.
* Observe and monitor the effects of psychotropic medications, including completeing the DISCUS assessment and the MOSES assessment every 6-months.
* Report significant side effects and DISCUS scores greater than 5 to the appropriate physician.
*Minimum Qualifications:*
* Current Nursing license in good standing (within state of MN)
* 1 year of experience using MDS 3.0
* BLS Certification
* Proficiency with computer software (MS Word, Excel, Outlook, EHR)
* Ability to contribute to interdisciplinary team framework
* Effective communication skills
*Preferred Qualifications:*
* Bachelor's of Science in Nursing (BSN)
* Experience working in a long term care facility
* 1 year of work experience as an MDS Coordinator
* Familiarity with RAI
