*Position Overview*
The Speech Therapist will provide *skilled evaluations and treatment* to home health patients with speech, language, cognitive, and swallowing disorders. SLPs may function as the *primary skilled discipline* and play a key role in improving patient safety, communication, and quality of life.
*Primary Responsibilities*
* Perform initial evaluations, treatments, re-evaluations, and discharges
* Develop and implement individualized plans of care
* Educate patients and caregivers on communication and swallowing safety
* Document accurately and timely per Medicare and agency standards
* Communicate effectively with physicians and interdisciplinary team members
*Qualifications*
* Current *Texas SLP license*
* ASHA CCC-SLP preferred
* Home health experience preferred
* *WellSky/Kinnser experience preferred*
* Strong documentation and communication skills
* Reliable transportation
*Service Area*
* Sherman and surrounding cities
* *Limited travel radius of approximately 25–30 miles from our Sherman office*

Pay: From $75.00 per hour
