We are privately owned, CHAP Accredited Home Health Agency based in San Jose, California. We provide care all throughout the greater Bay Area.
*We have an immediate need for per diem or part time to full time Physical Therapists in East and West Contra Costa County. *
Our founders include 150+ years of highly experienced nurses, healthcare administrators and other professionals in the post-acute industry dedicated to the highest quality clinical outcomes and patient satisfaction.

Compensation: Up to $75-80 per hour DOE in home health setting
*Responsibilities*
* Conduct thorough assessments of patients’ physical abilities and limitations.
* Develop individualized treatment plans based on patient evaluations and goals.
* Implement therapeutic exercises and modalities to promote recovery and rehabilitation.
* Provide education to patients and their families regarding treatment procedures and home exercises.
* Collaborate with other healthcare professionals to ensure comprehensive patient care.
* Monitor patient progress and adjust treatment plans as necessary.
* Participate in discharge planning to facilitate smooth transitions from inpatient to outpatient care or home community care.
* Maintain accurate documentation of patient progress and treatment outcomes.
*Experience*
* A degree in Physical Therapy from an accredited institution is required.
* 1+ year experience in a home health setting
* Knowledge of anatomy, physiology, and rehabilitation techniques is essential.
* Experience with acute care settings, pediatrics, or discharge planning is a plus.
* Strong interpersonal skills with the ability to communicate effectively with patients, families, and team members.
* A valid state license to practice physical therapy is required.

Pay: $70.00 - $80.00 per hour
