We are seeking a dedicated Licensed Practical Nurse (LPN) to join our healthcare team, providing high-quality patient care across various settings. The ideal candidate will have a strong foundation in patient assessment, medication administration, and clinical procedures, with experience in diverse medical environments such as long-term care, acute care, hospice, or hospital settings. The LPN will play a vital role in supporting patient recovery and well-being through compassionate and efficient nursing care, utilizing advanced EMR/EHR systems and adhering to HIPAA regulations. This position offers opportunities to work with specialized populations including geriatrics, pediatrics, individuals with developmental disabilities, and those requiring memory or dementia care.
*Duties*
* Conduct comprehensive patient assessments including vital signs, physical examinations, and health history documentation.
* Administer medications and injections accurately while following prescribed protocols.
* Perform IV infusions, catheterizations, tube feedings, and wound suturing as needed.
* Manage airway maintenance including ventilator support and airway management techniques.
* Assist with diagnostic evaluations such as blood draws (phlebotomy), specimen collection/processing, and imaging procedures.
* Provide specialized care for patients with complex needs such as Alzheimer's disease, dementia, or behavioral health conditions.
* Document all patient interactions thoroughly using EMR/EHR systems like Epic, Cerner, or Athenahealth while maintaining HIPAA compliance.
* Support discharge planning and coordinate with case management teams for post-acute or home & community care.
* Educate patients and families on health coaching topics including nutrition, medication management, and chronic disease management.
* Ensure infection control practices are strictly followed to maintain sterile techniques and aseptic procedures.
* Collaborate with multidisciplinary teams in various clinical settings such as outpatient clinics, inpatient units, or urgent care centers.
*Experience*
* Proven experience working in medical-surgical units, emergency rooms, ICU/CCU/PICU settings, or specialty areas like cardiology or neurology.
* Prior experience with critical care including Level I Trauma Centers or Level II Trauma Centers preferred.
* Familiarity with ICD-9/ICD-10 coding for accurate medical documentation and billing processes.
* Experience providing hospice or palliative care for terminally ill patients.
* Knowledge of pharmacology related to pain management, sedation protocols, and medication administration techniques.
* Proficiency in using EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks is highly desirable.
* Certification in Basic Life Support (BLS) required; additional certifications like ACLS or PALS are advantageous.
* Strong understanding of anatomy & physiology along with infection control standards to ensure safe patient environments. This Licensed Practical Nurse position is integral to delivering compassionate patient-centered care across multiple healthcare settings while advancing your clinical expertise through diverse experiences within our organization.
Pay: $30.00 - $38.31 per hour
