*Job Summary*
We are seeking a compassionate and dedicated Hospice Nurse to provide high-quality care to patients in their final stages of life. The ideal candidate will have a strong background in nursing, with experience in hospice and palliative medicine. This role requires excellent patient assessment skills and the ability to work collaboratively with families, caregivers, and interdisciplinary teams to ensure comfort and dignity for patients.
*Duties*
1. Provide direct care to Hospice patients.
2. Provide care by following the plan of care developed for each patient.
3. Report and discuss all medication changes to Patient Care Coordinator responsible for each patient
4. Communicate with co-workers and supervisor
5. Communicate with Patient Care Coordinator regarding changes in condition, new orders, etc.
6. Develop expertise in comfort care/needs for each individual patient.
7. Assist in the continuous changes/evolvement of Hospice Program
8. Responsible for clinical performance of hospice nursing to facilitate effective and efficient patient care
a. Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.
b. Attendance at bi-weekly IDT meetings
c. Follows up with obtaining lab results and notifying physician of any significant results as directed by Patient Care Coordinator
d. Provide assessments of patients and assist with nursing care and teaching of patient care and medication to the caregivers as necessary
e. Chart accurately and thoroughly in EMR the various tasks completed for and interactions that have taken place with each patient/ pt. family member as procedure requires within 24 hours of each visit
f. Complete on-call requirements and report on-call activity to the team
g. Adhere to documentation policies in discharges, eligibility, incident reports and measurable expected outcomes in the plan of care
h. Recognize/intervene/report abuse and/or neglect to proper resource
i. Fulfill on-call rotation schedule; RN is responsible for providing back up call when LPN is on-call
j. Destroys Narcotics/Medications at Our House as needed
k. Fills syringes of liquid medications at Our House as needed
9. Assists with bereavement to assure that the bereavement needs of the Hospice families are met
10. Represent Hospice of Murray County at Patient’s visitation or funeral
11. Assumes Patient Care Coordinator and Administrator responsibilities in his/her absences as assigned
12. Participates in Hospice of Murray County QAPI committee and assists in Performance Improvement Projects (PIPs)
13. Maintains regulatory requirements and knowledge of Hospice Medicare Conditions of Participation and Minnesota Hospice Rule
14. Participate in extra Hospice of Murray County related events (fund raisers, parades, etc.).
15. Other appropriate tasks that may arise.
Professional Development
* Complete annual Relias training modules.
* Complete required amount of continuing education credits to maintain licensure.

Job Type: Full-time
Pay: $52,359.61 - $63,056.73 per year
