We are seeking a medical professional, who lives the mission of 'Unstoppable Compassion' through their role as an attending physician of Long-Term Care. This position is open as of February 2026. At this time, we are requiring an attending physician for 65-75 residents.

Attending Physician Position Summary:
The attending physician is accountable to the Medical Director for fulfilling medical care and services to long-term care residents. This role involves working closely with the interdisciplinary team to ensure the highest standard of medical care, support quality improvement initiatives and uphold compliance with the Ministry of Long-Term Care regulations, while supporting the mission, vision and values of St. Joseph's Health System.
Reports to: Medical Director, through the LTC Administrator and COO

Key Responsibilities:
The Attending Physician shall be responsible for:
Assessing, planning, implementing and evaluating the residents medical care, develop a medical plan of treatment for the Resident and participate in the interdisciplinary approach to care.
Completing a medical history and physical examination within fourteen (14) days of the Resident's admission or in accordance with applicable legislation; and conducts a History and physical examination for each resident annually thereafter. Examination findings must be produced via electronic report in the resident's electronic health record, managed by the home.

All physicians will utilize the Home's electronic records (such as Point Click Care) to enter ALL medical related responsibilities to ensure all resident information is collected and monitored the same. Support and/or training will be provided as needed.

Completing medication, diet and treatment orders upon notification of the Resident's admission and visiting the Resident within fourteen (14) days of admission or in accordance with applicable legislation;

Reviewing the Resident's diet, medications, treatments and orders every three (3) months;

Visiting Residents as frequently as deemed necessary by the Resident's health status.

Shall conduct onsite rounds/visits for the residents for whom you are responsible physician where clinically indicated, as outlined below:
At a minimum:
o 1 – 75 residents – 1 on site visit per week;
o residents – 2 on site visits per week;
o Over 150 residents– 3 on site visits per week;
• Counter-signing all verbal orders/directions and laboratory reports, within fourteen (14) days;
• Comply with applicable provincial legislation;
• Providing an opportunity for the Resident/substitute decision-maker to participate fully in the development and revision of the Resident's medical care plan;
• Communicating with the Resident, their family or substitute decision-maker as appropriate and provide information to them regarding the Resident's health status. Documentation on the progress notes of such communication shall be completed;
• Referring Residents to the appropriate medical consultants and/or other health practitioners (i.e. physiotherapists, occupational therapists, social workers, speech therapists etc.,) when required;
• Where the physical or mental condition of a Resident is such that the Resident cannot properly be cared for in the Home, documenting such and attempting to arrange for the Resident to be admitted to a general hospital or an appropriate alternate facility that is best suited to provide the care required by the Resident, as decided upon collaboration with the Medical Director;
• Requisitioning diagnostic services as the Resident's condition warrants;
• Maintain records consistent with the College of Physicians and Surgeons of Ontario's record keeping requirements and any specific records required by regulation;
• Ensuring that all documentation, is current, clear, legible, dated, and signed;
• Participating in team discussions regarding advance directives with Residents and/or families/substitute decision-makers as required;
• Communicating with the Medical Director and or Administrator with respect to infectious disease surveillance in the Home.
• Ensuring education as stated in the FLTCA, 2021 section and section of the Regulations is completed based on educational need. This education can be completed through the home or another home, however, an attestation or a letter of confirmation from another facility, must be submitted to the home for our records.
• Shall provide or plan for on-call coverage and after-hours coverage per section 89(c)(iii) of the Regulations for the Residents;
• Shall, when not personally available to take a call, advise the Home of emergency medical services which may be utilized;
• Shall be entitled to time off for vacation or education, provided that arrangements are made for coverage for the needs of your residents;
• Shall, where possible, participate on committees as requested by the Medical Director or the Home's administrator (the "Administrator") (e.g. continuous quality improvement, medication management, infection control, professional advisory, medical records committees, etc.), so long as there is agreement between the Attending Physician and the Medical Director or Administrator that such participation is necessary or appropriate.
Qualifications:
• Medical practitioner licensed to practice in Ontario and a member in good standing with the College of Physicians and Surgeons of Ontario (CPSO)
• Experience as an Attending Physician in long-term care,
• Strong understanding of the LTC regulatory standards with experience in quality improvement and interdisciplinary team collaboration
• Experience with Point Click Care (PCC), Minimum Data Set (MDS) and Resident Assessment Instruments (RAI) is an asset
• A passion for caring and supporting those most vulnerable in our communities and those who call our campuses home
• Excellent interpersonal skills with the ability to build trust and engage meaningfully with residents, families and the care team.
