We are now hiring a _*Physical Therapist *_to become part of our rehabilitative team.
The therapist will provide evaluation and therapeutic services as needed by our patients in addition to the following:
* The successfully Hired individual will plan, develop, organize, implement and evaluate direct therapy in accordance with current rules regulations and guidelines that govern long term care facilities.
* Participate in discharge planning, implementation and development of residence care plans, residence assessments, etc.
* The therapist will also provide supervision and guidance to physical therapy assistants.
* Assist with educating family and staff to optimize patient outcomes and safety.
* Perform job duties in accordance with state and federal regulations and facility policies and procedures.
* Perform other job related duties as assigned or as needed.

*Requirements:*
* Bachelor's degree required
* Certified FL license in Physical Therapy
* Excellent communication skills
* Great clinical, personal, documentation and implementation skills
* Skilled nursing facility (SNF) experience preferred
* Ability to lead and mentor PTA staff members and work collaboratively as part of a interdisciplinary team.

Pay: $50.00 - $65.00 per hour
