Job Title: Case Manager

Program: Katoyiss Clan – Emergency Shelter

Department: Akoka'tssini Mental Wellness

Classification: Full-time – Term (fiscal year-end)

Shift: 75 Hours bi-weekly

Schedule: 8 AM – 4 PM; Monday – Friday

Rate of Pay: $25-$27 per hour, depending on experience.

The Opportunity

The Katoyiss Clan Emergency Shelter in Brocket—a 24-hour safe haven under Akoka'tssini Mental Wellness (AMW)—is looking for a dedicated Case Manager (CM) to help individuals on their path to healing.

At Katoyiss Clan, we operate on harm reduction principles, offering participants hot meals, snacks, showers, clean clothing, and compassionate support. As a Case Manager, you'll be a vital part of a multidisciplinary team, delivering trauma-informed care and personalized treatment plans. The CM will be providing referrals and transportation to recovery-oriented programs, ensuring every individual has access to the resources they need.

You'll advocate for participants, coordinate services, and build strong connections with families, community supports, and external agencies. Working closely with clinical and support staff, you'll help shape programs that align with our treatment model and empower participants on their healing journey.

About AMW

Akoka'tssini Mental Wellness (AMW) is a sub-department under the Aakom-Kiyii Health Services umbrella. AMW supports the Piikani community with services and programming directly related to Mental Health and Addictions. AMW's mission is to encourage Niitsitapi in their healing by providing continued services with integrity and kimmapiipitsin while reclaiming Piikanissini.

Role responsibilities and duties

  • Collaborate with participants to develop and monitor individualized goals, treatment and aftercare plans
  • Provide case coordination, advocacy, and referrals to internal and external services (e.g., mental health, housing, legal aid, cultural support)
  • Work closely with Elders, Knowledge Keepers, and cultural support workers to ensure services align with traditional values and practices
  • Maintain accurate, electronic case notes and confidential records in accordance with privacy laws and organizational policies
  • Participate in team meetings, case reviews, and interagency collaboration
  • Provide crisis intervention and emotional support as needed
  • Support intake and discharge processes with compassion and professionalism
  • Promote holistic healing approaches rooted in the physical, mental, emotional, and spiritual wellbeing of residents
  • Use Motivational Interviewing techniques to support resident readiness for change and empower self-directed healing
  • Maintain programming calendars, rosters, reports, and evaluation tools.
  • Track program outcomes and prepare reports for internal use, funders, or regulators.
  • Assess the needs and progress of individuals within the group
  • Facilitate open and supportive discussions about addiction and recovery
  • Create a safe, inclusive, and non-judgmental environment for group members
  • Provide transportation as needed
  • Assist participants with organizing personal affairs as requested
  • Attend/facilitated case consultation as needed
  • Participating in daily debriefing at shift change
  • Continuing professional development through ongoing education and training in addiction therapy Knowledge Skills and Abilities

  • Knowledge of Piikani/Blackfoot Culture and Language is an asset
  • Manage resident relationships and large caseload
  • Sound Knowledge and ability to demonstrate a practical application of the following techniques are considered an asset: crisis intervention, de-escalation skills, people management skills, conflict intervention and resolution.

Qualifications

  • Successful completion of a degree or diploma in a human services discipline, Social Work, Addiction Counselling, Indigenous Studies and/or related field
  • Minimum two years of work experience in addiction and/or mental wellness
  • Knowledge of trauma-informed, harm reduction, and culturally safe practices
  • Proven work experience as a Group Facilitator, Therapist, Counsellor, or similar role in a mental health setting
  • Understanding of the impacts of colonization, residential schools, and intergenerational trauma
  • First Aid, CPR, Applied Suicide Intervention Skills Training (ASIST), Non-Violent Crisis Intervention (NVDI), Trauma-Informed Care, Security, and WHMIS training are assets
  • Recovery Coach Training provided to the right candidate
  • Proficient in MS Office 365
  • Strong Organizational and communication skills

Assets

  • Lived experience or strong connection to Indigenous Culture and Language
  • Familiarity with traditional healing practices and ceremonies
  • Registration or eligibility for registration with a professional body (eg. RSW)
