*Position Overview:*
We are seeking a skilled and compassionate Physical Therapist to join our home health team. The PT will evaluate and provide individualized therapy services to patients in their homes, helping them regain mobility, improve strength, and maintain independence.
*Responsibilities:*
* Conduct patient evaluations and develop individualized care plans
* Provide therapeutic exercises, gait training, and mobility interventions
* Educate patients and caregivers on safe techniques and home exercise programs
* Document all patient care accurately in EMR (Kinnser/WellSky)
* Collaborate with physicians, nurses, and interdisciplinary team members
* Ensure compliance with agency policies and regulatory standards
*Qualifications:*
* Current Florida Physical Therapist license (required)
* Minimum 1 year of clinical experience (home health experience preferred)
* Strong clinical, problem-solving, and assessment skills
* Excellent communication and interpersonal skills
* Valid driver’s license and reliable transportation
