We’re expanding our rehabilitation team and looking for a compassionate, mission-driven Speech Language Pathologist (SLP) to provide _high-quality home-based care_ across Fairfield County. We provide skilled nursing, PT/OT/ST, and personal care services to individuals experiencing communication, swallowing, cognitive, developmental, or functional challenges. Your work helps patients stay safe, independent, and connected to their loved ones.
*What You’ll Do (Impact of the Role)*
As our Per Diem SLP, you will:
* Conduct comprehensive speech, language, cognitive-communication, and swallowing evaluations in patients’ homes
* Develop evidence-based, individualized treatment plans tailored to each patient’s goals
* Provide therapeutic interventions that support communication, safety, independence, and quality of life
* Educate patients, families, caregivers, and home staff on communication strategies and home programs
* Track progress, update goals, and collaborate closely with our nurses, PT/OT team, and physicians
* Maintain accurate, timely documentation in accordance with agency and regulatory standards
This role empowers you to use your clinical judgment, creativity, and compassion every single day.
*Qualifications*
To thrive in this role, you’ll need:
* Active Connecticut SLP license (CCC-SLP strongly preferred)
* Valid driver’s license and reliable transportation
* Experience in home health, outpatient, school-based, or clinical settings
* Strong understanding of communication, language, cognitive-communication, and swallowing disorders
* Confidence developing and implementing goal-driven treatment plans
* Familiarity with AAC devices and assistive communication strategies (a plus!)
* Excellent interpersonal skills—you’ll be working closely with families and interdisciplinary team members
*Coverage Area*
Fairfield County, CT — primarily Norwalk and nearby towns.
This is a road-based, patient-facing position providing care in the home.
Pay: $60.00 - $70.00 per hour
Expected hours: 10 – 25 per week
