About This Role
The RPN Care Coordinator plays a key role in coordinating care, supporting clients and families, and ensuring high-quality service delivery. This hybrid position combines the best of both worlds — administrative care coordination and direct client interaction through our Clinical Concierge program.
You'll act as the first point of contact for new clients, manage caregiver scheduling, and deliver real-time, empathetic support over the phone. You'll be part of a team that values compassion, professionalism, and excellence in every interaction.
Responsibilities
- Manage client schedules and coordinate services to ensure continuity and quality of care
- Communicate with clients, families, and caregivers to support their care journey
- Conduct intake assessments and triage calls through the Clinical Concierge program
- Assist in booking in-home initial assessments and wellness checkups.
- Conduct in-home wellness checkups and/or initial assessments, if required.
- Provide knowledgeable, compassionate telephone support and clinical guidance
- Support caregiver recruitment, onboarding, and performance feedback
- Maintain accurate documentation and uphold CareHop's care standards
- Contribute to quality improvement and Accreditation Canada readiness initiatives
Requirements
- Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario (CNO)
- 3+ years of experience in home care and/or acute care
- Experience in telemedicine, triage, or emergency care an asset
- Strong organizational and communication skills
- Exceptional empathy and professionalism in telephone interactions
- Demonstrate an ability to perform comprehensive clinical assessments both in person and virtually (over the phone) and make independent clinical decisions
- Proficiency in EHR systems and Microsoft Office
- Valid Ontario Driver's License
