*About Us:* Hilltop Home is a private, nonprofit, Intermediate Care Facility (ICF/IID) serving a maximum of 22 young children who have severe to profound developmental disabilities and are medically fragile. Hilltop Home provides a comprehensive residential program that includes individualized medical, therapeutic and developmental/educational services. The program is designed to assist young children with multiple disabilities and medical fragility to reach their fullest potential. The care includes medical, therapeutic and educational services provided in an intimate, homelike setting. Hilltop Home provides:
* Around-the-clock nursing care by a professional, caring staff
* Quality educational services through a certified, non-public residential school
* Physical, occupational and speech therapy
* A home-style environment with all the resources needed to care for special children/low staff to patient ratio
*Description:*
* Position facilitates development and rehabilitation of children with severe/profound cognitive, emotional, and physical disabilities by evaluating, diagnosing, and implementing treatment plans with appropriate and functional goals related to speech, language, cognitive-communication, and dysphagia. Protects all residents from abuse, neglect and exploitation.
* Evaluates speech, language, cognitive-communication, and dysphagia using standardized tools and/or clinical observation to develop treatment plans with functional goals based on a child’s specific needs in consultation with an Interdisciplinary Treatment Team (IDT).
* Creates, implements, and adapts relevant therapy plans to promote independence according to the child’s physical, cognitive, and emotional needs
* Provides documentation of each therapy session to track each child’s progress on treatment goals; recommending and implementing adjustments and modifications as needed.
* Completes discharge planning when appropriate in consultation with IDT.
* Contributes to Individualized Program Plan (IPP), Individualized Education Plan (IEP), and Peer Review meetings, by attending meetings, providing input on each child, and completing relevant documentation prior to each meeting
* Collaborates with IDT to promote carry-over of functional treatment goals and therapeutic plan outside of the therapy office
* Collaborates with relevant professionals (including primary care physicians, specialists, nurses, dieticians, other therapists, qualified professionals, teachers, etc.) as needed to insure the best treatment outcomes for the child
* Develops and provides training to staff during orientation on fostering a positive communication environment as well as using various relevant assistive technology devices
* Leads one-on-one feeding trainings with all staff who feed children by mouth to insure proper technique and to promote safety during the meal time routine
* Ensure all emergency procedures are followed in the event of a fire drill, actual fire, or other emergencies
* Adhere to organizations policies and procedures
* All other duties as required and requested by the supervisor
*Requirements:* Experience working with individuals with developmental disabilities preferred. Experience with feeding therapy a plus. One or more years' experience working with pediatrics and/or ICF/IID is preferred.
*Additional Requirements:* Solid interpersonal, communication (verbal and written) and organizational skills. Ability to reposition, lift, and transfer residents safely using proper techniques up to 40 pounds unaided, up to 100 pounds aided. Must submit to thorough pre-employment background check and drug screening.
*Education Requirements:* Master’s degree in speech language pathology from an accredited university. Certificate of clinical competency (CCC) from the American Speech-Language-Hearing Association required. License to practice in the state of North Carolina.
*Compensation is commensurate with experience. *
