*Position Summary: RN or LVN Charge Nurse*
As a *Charge Nurse (RN or LVN)*, you will play a vital role in providing high-quality care and promoting the well-being of each resident based on their individual care plans and assessments. This position is ideal for experienced, passionate nurses who want to make a meaningful difference in a skilled nursing facility setting.
*Key Responsibilities:*
* Monitor, assess, and document resident conditions during rounds.
* Implement and follow care plans as directed.
* Administer prescribed medications accurately.
* Participate in physician rounds and chart visits.
* Perform narcotic reconciliation between shifts.
* Identify significant changes in resident health and take appropriate action.
* Oversee and support nursing aides during your shift.
* Complete incident and accident reports as per policy.
* Conduct resident discharges in accordance with procedures.
* Complete nursing assessments before contacting physicians.
* Communicate clearly with residents and their families regarding health updates.
* Perform other tasks as assigned by the Director of Nursing or Administrator.
*Qualifications:*
*Education:*
* Graduate of an accredited LVN or RN program.
*Licensure:*
* Active LVN or RN license in *good standing in the state of Texas*.
*Experience:*
* Previous experience in a *skilled nursing facility* or long-term care setting preferred.
