We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including long-term care. As an LPN, you will work under the supervision of registered nurses and physicians, assisting with patient assessments, administering medications, and ensuring the comfort and well-being of patients.
*Duties*
* Provide direct patient care, including administering medications and treatments as prescribed.
* Conduct physical examinations and assess patient health status.
* Monitor vital signs and report any changes in patient conditions to the healthcare team.
* Assist with diagnostic evaluations and medical documentation.
* Perform procedures such as IV insertion, catheterization, and wound care.
* Collaborate with multidisciplinary teams to develop and implement individualized care plans.
* Educate patients and families on health management, medication administration, and post-discharge care.
* Maintain accurate medical records in compliance with HIPAA regulations.
* Participate in infection control measures and ensure a safe environment for patients.
* Engage in ongoing professional development to stay current with best practices in nursing.
*Requirements*
* Valid LPN license in the state of practice.
* Experience in various healthcare settings such as hospitals, nursing homes, or outpatient clinics is preferred.
* Familiarity with electronic medical record (EMR) systems like Epic or Cerner is a plus.
* Knowledge of medical terminology, anatomy, physiology, and pharmacology.
* Skills in critical care nursing, including experience with IV infusion, dialysis, or ventilator management are advantageous.
* Ability to perform triage and manage acute pain effectively.
* Experience working with individuals with developmental disabilities or in memory care settings is beneficial.
* Strong communication skills to interact effectively with patients, families, and healthcare professionals.
* Basic life support (BLS) certification is required. Candidates should be prepared to demonstrate their commitment to providing exceptional patient care while adhering to established protocols and standards.
