*What You’ll Do as a Physical Therapist*
* Provide intermittent skilled physical therapy in patient homes per physician orders.
* Conduct initial assessments, create care plans, and revise treatment as needed.
* Supervise Physical Therapy Assistants (PTAs) and Home Health Aides (HHAs) to ensure continuity and quality.
* Document evaluations, OASIS assessments, progress notes, and discharges in a timely manner.
* Collaborate with physicians and the interdisciplinary care team on treatment plans and patient updates.
* Educate patients and caregivers on home exercise programs and safety.
* Attend staff meetings, in-services, and participate in quality improvement efforts.
* Travel locally to patient homes and maintain accurate records in accordance with agency policies.
*What We’re Looking For*
* Graduate of an accredited Physical Therapy program.
* Active PT license in Colorado.
* At least 2 years of clinical experience preferred.
* Valid CPR certification.
* Reliable transportation and valid auto insurance.
* Strong communication, documentation, and clinical reasoning skills.
* Independent, self-motivated, and committed to patient-centered care.
