About
The Physical Therapist is responsible for evaluating, planning, and implementing individualized therapy programs for residents to improve mobility, relieve pain, and prevent or limit permanent physical disabilities. The PT works as part of an interdisciplinary team to enhance residents’ functional independence and quality of life.
*Key Responsibilities*
* *Assessment & Evaluation*
* Perform comprehensive physical therapy evaluations upon admission and as needed.
* Develop and update individualized treatment plans based on assessment findings and physician orders.
* *Treatment Implementation*
* Administer physical therapy treatments to improve strength, mobility, balance, and functional abilities.
* Utilize therapeutic exercises, gait training, modalities, and manual therapy as appropriate.
* *Documentation & Compliance*
* Accurately document evaluations, daily notes, progress reports, and discharge summaries in accordance with state, federal, and facility guidelines.
* Ensure compliance with Medicare, Medicaid, and third-party payer requirements.
* *Interdisciplinary Collaboration*
* Participate in care plan meetings and collaborate with nursing, occupational therapy, speech therapy, and social work to ensure comprehensive resident care.
* Educate residents, families, and caregivers on therapy goals and techniques for home continuation.
* *Safety & Quality Assurance*
* Adhere to infection control protocols and maintain a safe treatment environment.
* Monitor residents’ responses and adjust treatment plans as needed.
*Qualifications*
* Graduate of an accredited Physical Therapy program.
* Current state license as a Physical Therapist (PT).
* Experience in long-term care or geriatric setting preferred.
* Strong communication and documentation skills.
* Ability to work collaboratively as part of an interdisciplinary team.
Job Types: Full-time, Part-time, PRN, Per diem
Pay: $40.00 - $52.00 per hour
Expected hours: 37.50 per week
