Job Title: Occupational Therapist

Job Summary:

We are seeking a qualified Occupational Therapist to join our home health team. The Occupational Therapist will work with patients in their homes to help them improve their ability to perform daily activities and reach their maximum level of functioning. The ideal candidate will have experience working with patients of all ages and backgrounds, and a passion for helping others achieve their goals.

Responsibilities and Duties:

• Conduct initial patient evaluations to assess their functional abilities
• Develop individualized treatment plans to address each patient's unique needs
• Provide therapeutic interventions and activities to improve patient's strength, mobility, and coordination
• Teach patients and their families how to safely and independently perform daily activities
• Collaborate with other healthcare professionals to ensure comprehensive care for patients
• Document patient progress and update treatment plans as needed

Qualifications and Skills:

• Bachelor's or Master's degree in Occupational Therapy
• Valid Occupational Therapy license in the state of (state)
• Previous experience working in a home health setting preferred
• Strong communication and interpersonal skills
• Ability to work independently and manage a caseload of patients
• CPR certification
