Accountable for team productivity and the timely completion of delegated work assignments in accordance with established policies and standards. Participates and/or approves the evaluation of personnel.
Assist in initiation, completion of MDS and other facility, state and federal required assessment forms. Coordinate residents' level of care and assessment schedules with other departments within the facility, therapy providers and consultants. Assist with the initiation and completion of residents' plan of care. Assist with staff education and development of facility forms and tools to ensure accurate assessment of residents' needs. Obtain, distribute and review Q.M.'s monthly from the State. Update PCC as need and assign passwords for staff. Monitor Nursing documentation ensure completeness and accuracy. Requirements Educational Attainment • Graduate of accredited RN School of Nursing. • Bachelor of Science in Nursing. Licensure/Certification • Current Ohio Nursing License. • Certification in Gerontology. Specialized Knowledge/Skills • Working knowledge of CMS and ODH regulations governing care of nursing home residents. Training in the completion of the MDS, RAP instrument and the correction procedure. Knowledge of Medicaid, Medicare and HMOs
Kind & Length of Experience • 3-years Geriatric experience.
3-years experience with all components of the MDS process, including submission to the State.
