The Pharmacy Medication History Technician (MHT) is responsible for obtaining and documenting patients' current prior to admission medication(s) into the hospital's electronic medical record, ensuring the information is accurate and up to date for use by providers, nurses, and pharmacists during the patients' admission, discharge, and transition of care. Responsibilities:
Interviews patients, family members, guardians, etc. to obtain complete medication histories. Communicates with providers, retail pharmacies, living facilities, etc. for additional medication history information and verify accuracy. Documents medication histories in the patient's electronic medical record. Notifies pharmacist(s) of any discrepancies or potential patient safety issues identified in obtaining the medication histories. Communicates with staff and other care providers regarding assigned duties. Responds to and triages requests from customers for assistance with pharmacy services such as medication delivery, drug information, etc. Operates and supports automated dispensing technology to supply correct medications for individual patients, according to pharmacist and provider orders. Orders, receives, restocks and rotates inventory, and completes expiration date management. Contributes to process improvement projects. Completes regulatory compliance inspection of medication storage areas, including inspections and/or controlled substance inventories, and other regulatory documentation as required. Adheres to controlled substance policies and all policies and procedures of the department. Ensures accurate dispensing and documentation of controlled substances. Ensures necessary safety and quality control in the medication use system. Is available and flexible to work varied schedules as required to ensure coverage for the area.
Education: High school diploma or equivalent required and completion of a certified pharmacy technician training program Licensure/Certification: Registered as a Pharmacy Technician with the North Carolina Board of Pharmacy within 30 days of employment Pharmacy technician certification from the Pharmacy Technician Certification Board (PTCB) required. Ability to obtain PTCB Medication History Certificate. Experience: Prior experience in a hospital or retail pharmacy preferred. Knowledge, Skills and Abilities: Knowledge of drug names, medical terminology. Basic math skills. Excellent interpersonal skills and the ability to deliver excellent customer service to teammates, patients, supervisors and providers. Attention to detail and a good work ethic. Ability to prioritize responsibilities in a high-volume environment. Ability to read, write and communicate effectively in English. Proficient with MS Office and able to learn new pharmacy software rapidly. Ability to tolerate latex products and exposure to hazardous drugs. Ability to stand or walk on uncarpeted floors and work in close conditions for duration of shift. Valid NC Driver's License: No If driving a Wayne UNC Vehicle, must be 21 years old and MVR must be approved by Risk Management.
