Current State License as a Physical Therapist Proof of current CPR certification Current driver's license. Must have reliable transportation and insurance 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting Previous home care experience - Preferred Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program
Roles & Responsibilities
Perform clinical assessments to identify patient needs and appropriateness for therapy. Document and review Plans of Care (POC), progress notes, and other clinical records on an ongoing basis. Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referrals and assessments. Instruct patients and families in safe transfer techniques and the use of assistive devices. Educate patients and families regarding therapeutic procedures appropriate to the patient's age, condition, culture, and learning readiness. Ensure compliance with infection control and universal precautions during patient care activities.
