Conduct comprehensive evaluations for patients in both inpatient and outpatient settings, crafting personalized treatment strategies Implement physical therapy interventions targeting mobility, strength, balance, and overall function Collaborate closely with nurses, physicians, case managers, and rehabilitation experts to enhance patient outcomes Instruct patients and their families on therapy objectives, discharge procedures, and at-home exercises Accurately document evaluations, treatments, and patient progress in line with facility and regulatory protocols Engage in interdisciplinary care team discussions and case rounds to ensure seamless coordination of care Education
Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution Certifications
State licensure as a Physical Therapist CPR certification is preferred Additional specialized certifications (e.g., neurological, geriatric) are advantageous, but not essential Skills
Prior experience in a similar role is highly desirable Ability to work with patients presenting intricate medical and mobility challenges Exceptional communication and collaborative skills Proficient with digital documentation and clinical charting systems Benefits
