*Fifth Star Care Management *is seeking a dedicated and compassionate *Speech Language Pathologist (SLP)* to join our *IN-HOUSE* therapy team! New Grads Welcome!
_*Open to Part-Time and PRN as well!*_
_*$10,000 sign-on and re-location bonus for Full-time position!*_
*Responsibilities*
* Evaluate, diagnose, and develop individualized treatment plans for residents with speech, language, cognitive-communication, and swallowing disorders.
* Deliver evidence-based therapy services in accordance with physician orders and facility policies.
* Collaborate closely with an interdisciplinary team, including Physical Therapists, Occupational Therapists, Nurses, and Physicians, to ensure comprehensive care.
* Educate patients, families, and caregivers on treatment goals, home programs, and progress.
* Maintain accurate and timely clinical documentation in compliance with state, federal, and facility guidelines.
* Participate in care plan meetings and contribute to discharge planning as needed.
*Qualifications*
* Master’s Degree in Speech-Language Pathology from an accredited program.
* Current *State Licensure (or eligibility for licensure)* as a Speech-Language Pathologist.
* Current *Certificate of Clinical Competence (CCC-SLP)* from ASHA (CFY candidates welcome to apply).
* Prior experience in a Skilled Nursing Facility or similar setting preferred, but not required.
* Strong interpersonal, organizational, and communication skills.
