The Health Home Data Analyst (Internal title: Health Home Care Coordinator) plays an important role in supporting high-quality, client-centered care within the Health Home program through accurate data management, strong communication, and reliable coordination across teams. This position helps ensure care teams have timely, organized information needed to support individuals receiving services. By maintaining healthcare tracking systems, validating data, and generating reports, this role contributes to program effectiveness, operational efficiency, and compliance.

Key Responsibilities
Maintain and update healthcare tracking systems to ensure information is accurate, current, and accessible
Verify and cross-reference data across multiple platforms to support workflows, reporting, and compliance
Generate and organize reports to support care coordination activities and program trackingCommunicate professionally with internal teams to support collaboration and continuity of care
Identify and elevate discrepancies or unclear information to ensure accuracy and consistency
Support special projects, operational improvements, and department initiatives
Participate in required meetings, trainings, and organizational events
Perform additional duties as assigned by department leadership
Requirements, Skills, Knowledge and Expertise
High School/GED required
WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS
Strong communication and customer service
Attention to detail
Basic computer literacy – knowledge and experience with Excel preferred
Ability to follow processes and protocols
