VNS Health is seeking a dedicated Physical Therapist to provide high-quality home care services to adult and geriatric patients throughout the Bronx. As part of our Healthcare & Medical Services team, you will deliver evidence-based physical therapy in patients’ homes and community settings, helping them regain function, improve mobility, and maintain independence. Working closely with nurses, social workers, physicians, and caregivers, you will perform comprehensive assessments, develop and update individualized plans of care, and set measurable functional goals that reflect each patient’s medical condition, home environment, and personal priorities. You will provide therapeutic exercises, gait and balance training, transfer training, neuromuscular reeducation, pain management strategies, and patient and caregiver education, while monitoring response to treatment and adjusting interventions as needed. Documentation will be completed in a timely and accurate manner using electronic health records, in accordance with professional, regulatory, and VNS Health standards. In this role, you will also identify safety risks in the home, recommend adaptive equipment and home modifications, and coordinate with the interdisciplinary team to ensure seamless, patient-centered care transitions.
