Oak Street Health is seeking a compassionate, patient-centered Primary Care Physician (Spanish Required) to join our Healthcare & Medical Services team. As a core member of our primary care model for adults on Medicare, you will provide comprehensive, longitudinal care to patients in underserved communities, focusing on prevention, chronic disease management, and improved health outcomes. You will manage a dedicated panel of adult patients, conduct thorough evaluations, develop individualized care plans, and coordinate care with a multidisciplinary team including nurses, medical assistants, care managers, behavioral health specialists, and social workers. Fluency in Spanish is essential, as you will communicate directly with a predominantly Spanish-speaking patient population, ensuring patients fully understand their conditions, medications, and care plans. In this role, you will provide same-day, scheduled, and follow-up visits in a clinic-based setting, perform routine primary care procedures, review diagnostic tests, and leverage data and population health tools to proactively close care gaps. You will collaborate with specialists and community resources to support patients’ medical and social needs, advocate for high-quality, evidence-based care, and contribute to continuous quality improvement initiatives.
