Primary Care Physician

Oak Street Health is seeking a compassionate and mission-driven Primary Care Physician to join our Healthcare & Medical Services team, providing high-quality, comprehensive primary care to adult patients on Medicare in underserved communities. As a Primary Care Physician at Oak Street Health, you will build long-term relationships with a dedicated patient panel, focusing on preventive care, chronic disease management, and coordinated, team-based treatment that improves health outcomes and reduces avoidable hospitalizations. In this role, you will deliver evidence-based primary care through office visits, same-day appointments, and follow-up care, performing thorough histories, physical exams, and diagnostic evaluations. You will develop individualized care plans that address medical, behavioral, and social needs, working closely with nurses, care managers, social workers, behavioral health specialists, and other members of an interdisciplinary team. You will use our integrated electronic health record to document encounters, track quality metrics, and coordinate care across settings. Your responsibilities will include managing common and complex chronic conditions such as diabetes, hypertension, COPD, heart failure, and depression; providing preventive screenings, immunizations, and wellness visits; and addressing acute issues in a timely, patient-centered manner. You will practice thoughtful, value-based medicine by emphasizing education, shared decision-making, and proactive outreach to keep patients healthy and engaged in their care.
