BreakThrough Physical Therapy is seeking a compassionate, patient-centered Physical Therapist PRN to join our Healthcare & Medical Services team. In this flexible, as-needed role, you will deliver evidence-based physical therapy interventions that improve mobility, function, and pain relief for a diverse patient population. As part of a collaborative, multidisciplinary environment, you will work closely with referring providers, support staff, and fellow therapists to achieve exceptional clinical outcomes and an outstanding patient experience.
The Physical Therapist PRN will perform comprehensive evaluations, establish individualized plans of care, and implement treatment programs using a wide range of therapeutic exercise, manual therapy, functional training, and pain management strategies. You will educate patients and families on diagnosis, home exercise programs, self-management techniques, and strategies to prevent re-injury, ensuring each patient is empowered to participate actively in their recovery.
This position is ideal for a clinician who values evidence-based practice, innovation, and continuous improvement. You will utilize standardized outcome measures, document accurately and efficiently in our electronic medical record system, and adjust treatment plans based on patient response and current clinical guidelines. You will collaborate with the care team to coordinate services, communicate progress, and support safe, timely transitions in care.
