Centennial Peaks Hospital has been providing psychiatric services to the Denver-metro area for more than 30 years. Located in beautiful Louisville, CO, our 104-bed facility provides a peaceful setting for those seeking treatment for mental illness including chemical dependency. Centennial Peaks offers unique and individualized programming for adolescents and adults that sets us apart from many other treatment facilities. Our dynamic leadership team includes several Licensed Social Workers and a seasoned nursing department. Our hospital is fortunate to provide Electroconvulsive Therapy, Intensive Outpatient and Partial Hospitalization Programs in addition to inpatient services.

Master's degree in Social Work, Clinical Counseling, Psychology, or Social Services related field REQUIRED

Our Social Workers provide clinical social work services on each of our five units (general adult mental health, adolescent, two intensive treatment units and a detox/dual diagnosis unit). Our Social Workers practice from a trauma informed, strengths based perspective and work with our patients and their support systems to set them up for success in the hospital and after discharge. Our Social Workers are responsible for psychosocial assessments, daily process group, participation in daily treatment planning, processing mental health certifications, crisis intervention, and discharge planning/case management services with referral sources and other departments within the hospital. Our Social Workers are a vital part of the treatment team and provide a balanced and patient centered approach to care. This is a dynamic, fast-paced position with many opportunities for growth and development. An ideal candidate will be self motivated, organized, able to multitask and comfortable with crisis.

Job Duties:

  • Provide patients and families with assessment, interventions, support, advocacy, and case management as a primary provider and/or as ancillary support to the primary medical provider.
  • Practices as a member of multi-disciplinary team, in the provision of clinical intervention and support to patients and families.
  • Assesses, develops treatment plans, and intervenes to address the needs identified in the assessment and by the Team.
  • Utilizes knowledge of culture, age, and developmental stages to develop treatment plans and provide interventions, either as an independent practitioner or as a member of a clinical team.

This is a Tuesday-Saturday position

Supervision towards licensure available.
