Physical Therapist

Position Summary

The Physical Therapist (PT) / Physical Therapist Assistant (PTA) provides skilled physical therapy services to residents within a skilled nursing facility. The PT delivers evaluations, establishes treatment plans, and oversees care, while the PTA implements treatment interventions under the supervision and direction of a licensed Physical Therapist. Together, they work to improve residents' mobility, strength, balance, and overall functional ability, promoting independence and quality of life.

Key ResponsibilitiesPhysical Therapist (PT):

• Evaluate residents and develop individualized physical therapy treatment plans based on clinical findings and functional goals.
• Provide direct physical therapy services, including therapeutic exercise, gait training, balance training, and functional mobility activities.
• Supervise and direct Physical Therapist Assistants (PTAs) and therapy aides in accordance with state regulations.
• Monitor resident progress, modify treatment plans as needed, and ensure appropriate clinical outcomes.
• Complete evaluations, progress notes, discharge summaries, and other required documentation accurately and timely.
• Educate residents and caregivers on safety, mobility techniques, and home exercise programs.
• Participate in interdisciplinary care planning and team meetings.
• Ensure compliance with all state, federal, and facility regulations and professional standards.

Physical Therapist Assistant (PTA):

• Carry out physical therapy treatment plans developed by the Physical Therapist to address residents' mobility and functional needs.
• Deliver direct therapy services, including therapeutic exercises, gait training, and balance activities, in a safe and effective manner.
• Observe, document, and report resident responses to interventions, including progress and changes in condition, to the supervising PT.
• Assist with resident assessments and contribute to care planning as appropriate.
• Educate residents and caregivers on exercises and techniques to promote safety and carryover outside therapy sessions.
• Maintain accurate, timely, and compliant documentation in the electronic medical record system.
• Ensure proper use and care of therapy equipment and assistive devices.
• Collaborate with interdisciplinary staff to support coordinated resident care.

QualificationsFor Physical Therapist (PT):

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the state of practice.
• Strong clinical, communication, and documentation skills.
• Experience in a skilled nursing facility or geriatric setting preferred.

For Physical Therapist Assistant (PTA):

• Graduate of an accredited Physical Therapist Assistant program.
• Current PTA license in the state of practice.
• Strong communication, clinical judgment, and documentation skills.

Pay: $ $60.00 per hour

Expected hours: 40 per week

Experience:

• Physical Therapist/Physical Therapist Assistant: 1 year (Preferred)

License/Certification:

• BLS Certification (Required)
• Licensed Physical Therapist/Physical Therapist Assistant (Required)
