Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by checking vital signs and observing changes in condition; report findings accurately and promptly
- Administer medications following care plans and HIPAA regulations to ensure safety and compliance
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking
- Use specialized equipment such as Hoyer lifts or resident's to ensure safe transfers and mobility support
- Maintain detailed documentation of care provided, patient observations, and any incidents in accordance with privacy standards
- Support clients with memory care needs such as Alzheimer's or dementia by implementing personalized routines and engaging activities
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment in compliance with health standards
- Collaborate with healthcare professionals to develop and follow individualized care plans that meet each client's unique needs
- Provide emotional support and companionship to foster social engagement and reduce feelings of isolation
Skills
- Proven experience in home & community care or assisted living environments, especially with seniors or individuals with disabilities
- Knowledge of first aid, CPR, and emergency response procedures to ensure client safety at all times
- Familiarity with working in group homes, nursing homes, or hospice settings is highly desirable
- Strong patient observation skills to detect changes in condition or behavior promptly
- Ability to lift heavy objects safely using equipment like Hoyer lifts; physical stamina for extended periods of caregiving tasks
- Experience working with people with developmental disabilities, autism, or memory-related conditions such as Alzheimer's disease or dementia
- Excellent communication skills for effective collaboration with clients' families and healthcare teams
- Understanding of HIPAA regulations to protect client confidentiality at all times
- Compassionate approach combined with patience, adaptability, and a genuine desire to improve clients' quality of life
