Responsibilities May Include:
Clinical Oversight: Oversees patient care delivery, providing feedback and ensuring it meets professional standards and protocols through support visits and audits.

• Initial Home Visit Assessments
• In Home Care Conferences with clients/ family and Government Funder.
• In Home Delegated Task training
• In Home PSW support visits
• In office PSW interview/ orientation
• In office Performance Development meetings

Staff Management: Assist in recruiting and retaining PSWs, providing education and guidance, addressing conflicts and promoting effective communication.

Professional Development: Acts as a mentor and educator for PSWs, providing feedback, sharing best practices and helping staff grow in their skills and knowledge

Systems Improvement: Works to enhance healthcare delivery systems by enforcing policies and providing education to PSWs.

Administrative Link: Serves as a crucial link between Government Funder and the front line staff.

Qualifications:
Must possess a Registered Practical Nursing (RPN) Diploma and be in good standing with the CNO.

Experience in Home Care is an asset

Bilingualism is an asset

Must Demonstrate effective leadership and communication skills

Must be able to operate with cultural sensitivity and competence in their practice

Must serve as a ethical model for staff.
