We are seeking a dynamic and experienced Lead Physical Therapist to join our healthcare team, dedicated to delivering patient care in the Assistive Senior Living Community. In this pivotal role, you will a be leading physical therapy treatments to maintaing and improve residents balance to support aging in place . Your expertise will be greatly appreciated in esnhancing and creating new care programs. *Responsibilities*
* Evaluations of referred to PT patients and creation of care plans to promote improved strength balance preventing falls.
* Home environment assessments and consultation for safety.
* Collaborations with community leadeship to best serve patients.
* Timely minimal charting with known EMR.
*Requirements*
* Valid licensure as a licensed Physical Therapist with relevant state or regional credentials
* Proven experience in acute care hospital settings, outpatient clinics, or home health environments with a strong foundation in inpatient and outpatient care
* Demonstrated expertise in discharge planning, care plan development, and patient education
* Knowledge of physiology, anatomy, medical terminology, and rehabilitation techniques
* Familiarity with EMR systems for documentation and patient management
* Experience working with eldery populations including in settings such as hospitals or community clinics
* Strong leadership skills with the ability to motivate residents and coordinate treatments

Pay: $55.00 - $60.00 per hour
