Provide high-quality, individualized care to our residents, including administering medications, performing wound care, and assisting with daily living activities Develop and implement personalized care plans in collaboration with the interdisciplinary team Participate in ongoing assessments and evaluations to ensure resident needs are being met Communicate effectively with residents, families, and healthcare team members to ensure seamless care Maintain accurate and detailed records of resident care and services Requirements:
Current licensure as a Licensed Practical Nurse (LPN) in the state of Wisconsin Ability to provide high-quality care with minimal supervision Strong communication and interpersonal skills Ability to work effectively in a fast-paced environment Passion for delivering exceptional patient care and experience working with older adults Why Join Our Team:
