As a Hospice Home Health Aide, you are more than a caregiver – you are a cornerstone of comfort, providing essential support that allows our patients to live their final chapters with dignity in their own homes. Every day, you will provide essential, compassionate hands-on care, ensuring dignity and well-being for our patients. To be successful in this role, you will possess a dedicated spirit of service, strong communication skills, a meticulous approach to patient support, and an unwavering commitment to our mission of dignified, high-quality healthcare. Responsibilities:
Personal Care & Comfort: Assisting with personal hygiene, mobility (ambulation and transfers), and promoting overall patient well-being. Patient Monitoring: Diligently reporting any changes in condition to the nursing team. Supportive Environment: Performing light housekeeping, preparing meals, and ensuring a safe, healthful, and comforting home environment.
Job Requirements:
High School Diploma or equivalent required. Certified Nurse Assistant (CNA) in the state of Minnesota (MN) required. Basic Life Support (BLS) from the American Heart Association (AHA) required. Must have a valid Driver's License. Must have current vehicle insurance.
