This scope of work is for Program Manager (2. Manager) services for the development of new and existing functionalities required to support the Tobacco Free Florida Cessation Interventions health systems projects and technical solutions including the Tobacco Free Florida AHEC Cessation Tracker (TFFACT), E-referral hub/functionality, Florida Clean Air Act system (FCAA system), as well as the Quitline. Contractor will provide these services to the Florida Department of Health (Department).
TERM This scope of work will begin on 1/1/2026 or the date on which the purchase order is issued, whichever is later. It will end at midnight, Eastern Time on 6/30/2026. The State of Florida’s performance and obligation to pay under this purchase order and any subsequent renewal is contingent upon annual appropriation by the Legislature and satisfactory performance of the Contractor.
CONTRACTOR QUALIFICATIONS AND EXPERIENCE
Minimum of 25 years of experience collaborating with health care providers including hospitals, Federally Qualified Health Centers (FQHCs), free and charitable clinics, behavioral health substance abuse treatment centers, health insurance plans provider groups, large physician organizations, and accountable care organizations.
Minimum of 20 years of experience coordinating with local, state, and federal organizations to facilitate systems change within health systems.
Minimum of 10 years of experience with health systems including coordination, facilitation, and implementation of various information technology solutions.
Minimum of 10 years of experience facilitating training efforts in health systems change.
Minimum of 20 years of experience facilitating systems change activities and efforts.
Minimum of 10 years of experience with financial management related to health care systems.
Minimum of 3 years of experience with health care and cessation evaluation findings, implementation of evaluation findings into system change efforts.
Minimum of 10 years of experience with health plans and health plan providers.
Minimum of 25 years of experience with health system types and principles of health systems change.
Minimum of 10 years of experience identifying and documenting trends related to national and state health policy initiatives.
Minimum of 15 years of experience with partnership building and relationship management in the health care field.
Minimum of 15 years of experience with program development and process improvement measures.
Minimum of 10 years of experience with strategic planning.
Minimum of 15 years of experience with technology systems and solutions related to health care systems including cessation.
Minimum of 3 years of experience with tobacco cessation initiatives and systems.
Minimum of 10 years of experience leading enterprise-wide, large and small teams with focus in healthcare environments.
Minimum of 10 years of experience in project and program management with focus in healthcare environments and health systems change.
CONTRACTOR RESPONSIBILITIES
Provide Program Manager services with qualified staff assigned within specified timeframes.
Develop Department Cessation program strategy and high-level plans in collaboration with the Department.
Hold monthly meetings with Department staff, local AHEC offices, and leadership to advise, status, and facilitate systems change; document meetings and include at least one in‑person meeting per quarter.
Participate in vendor calls with the Quitline vendor to resolve cessation, systems change, or interface issues.
Participate in meetings with Department evaluators, incorporate evaluation suggestions into system change, and coordinate participation of Department’s Cessation staff with State and Community Interventions and Health Communications staff.
Assess impact of health and policy changes (e.g., HHS reorganization, professional associations, voluntary health organizations) on tobacco cessation.
Identify and document national and state health policy trends that might affect health system financing or operations, especially those impacting cessation.
Partner and collaborate with Department Cessation program management, BTFF programs, and senior leadership in the healthcare and business community to communicate program strategy and identify opportunities using health systems change and information technology solutions.
Develop comprehensive lists of health care systems and provider groups, including key information; maintain and update these lists throughout the term, and submit upon request.
Identify champions in local health organizations (e.g., executives, directors, coordinators) and engage them in systems change activities.
Contact and coordinate with health care provider groups to determine willingness to engage in system changes and structure activities.
Coordinate with local, state, and federal organizations (e.g., AHCA, DCF, DOEA, HHS, CDC, NIH, HRSA, SAMHSA) to facilitate systems change within health systems.
Coordinate with major EHR vendors (Epic, Oracle, Cerner, Meditech, AllScripts, Veradigm, NexGen, AthenaHealth, eClinicalWorks) to support e‑referrals.
Conduct orientation, training, and mentoring of Department staff and section leads on principles of health system types and health systems change.
Coordinate creation of program and user manuals.
Lead or participate in health systems change trainings for Tobacco Treatment Specialists.
DATA SECURITY AND CONFIDENTIALITY The Contractor, its employees, subcontractors, and agents must comply at all times with all Department data security procedures and policies per the attached Data Security and Confidentiality document.
DELIVERABLES
Monthly: Provision of Program Manager services as specified.
METHOD OF PAYMENT A purchase order will be issued to the Contractor. Payment will be fixed price. The Contractor will not receive payment in advance. Invoices will be submitted monthly with detailed accounting of deliverables. The Contractor is responsible for all tasks and deliverables.
PERFORMANCE MEASURES AND FINANCIAL CONSEQUENCES All deliverables must be completed 100%. Failure to submit Deliverable 5.2.1 will result in a 5% reduction of the monthly invoiced amount. Failure to comply with Data Security and Confidentiality will result in a 10% reduction for each non‑compliant month.
TRAVEL REIMBURSEMENT The Contractor will not be reimbursed for any travel expenses.
STAFF REQUIREMENT All Contractor staff must meet or exceed the qualifications stated above. The Department reserves the right to review and approve replacement staff.
