Overview The Mobile Driver/Outreach Worker supports the delivery of mobile clinical and outreach services to individuals experiencing homelessness. This role is responsible for safely operating the mobile clinic and assisting with day‑to‑day field operations, while also engaging clients in outreach settings and helping connect them to care and services. The Mobile Driver/Outreach Worker plays a key role in maintaining efficient workflow, supporting the clinical team, and ensuring a consistent and welcoming presence in the community.
Key Role Responsibilities
Safely operate and transport the mobile clinic to scheduled service locations; perform basic vehicle checks and report maintenance concerns as needed
Assist with mobile clinic setup and breakdown, including equipment, supplies, and site readiness
Support client flow on the mobile clinic and in outreach settings, including registration, EVS verification, patient check‑ins/check‑outs, referral processing, service coordination, documentation support, and other administrative duties related to outreach and clinical operations.
Conduct outreach in community settings (e.g., encampments, shelters, meal programs) to engage individuals and provide information about available services
Distribute basic needs items and resource materials in alignment with program guidelines (e.g., hygiene kits, food, clothing, water)
Provide basic client support and linkage to care, including referrals and follow‑up coordination as directed by the care team
Accurately document outreach encounters, services provided, and client interactions in HMIS (Homeless Management Information System), AthenaOne and other required systems in a timely manner
Enter and maintain required HMIS data elements (e.g., demographics, service connections, referrals, and engagement activities) in alignment with program and grant requirements
Assist in tracking and re‑engaging clients who have disengaged from care, including documenting outreach efforts in HMIS
Collaborate with clinical and outreach staff to ensure smooth daily operations and continuity of care
Maintain awareness of surroundings and support a safe environment for staff and clients in field settings
Adhere to all organizational policies and regulatory requirements, including HIPAA, OSHA, infection control standards, and mobile clinic protocols
Support proper handling, transport, and storage of vaccines and other temperature‑sensitive medications in accordance with cold chain requirements, including ensuring timely transfer to appropriate refrigeration units and immediately reporting any temperature excursions
Support Environment of Care (EOC) standards by maintaining a clean, organized, and safe mobile clinic environment
Knowledge, Experience, and Skills Formal Education and Training
High school diploma, GED or equivalent required
Associate’s degree in Human Services preferred
Valid driver’s license with clean driving record required; personal reliable transportation highly preferred.
Experience
At least 2 years of experience reaching out to vulnerable individuals who are often the most difficult to serve
At least 1 year of experience in connecting individuals to essential services including healthcare, mental health, and addiction services
Skills
Ability to relate effectively to homeless individuals who are at high risk for HIV and other communicable diseases.
Knowledge of Baltimore City community resources for addiction, social services and mental health
Well‑developed interpersonal skills necessary to engage clients and promote positive relationships with other community agencies and providers
Able to work as part of an interdisciplinary team
Able to take initiative and problem solve
Able to work with ill, disabled, emotionally upset, and sometimes hostile clients
Able to provide outreach services via walking, car, or mass transit
