Responsibilities Patient Care & Clinical Services
- Direct Patient Care: Intervene as appropriate to improve prescribing, administration, and monitoring practices within the facility.
- Medication Therapy Management (MTM): Provide MTM services and patient education as defined in the Illinois Pharmacy Practice Act.
- Dosing Recommendations: Determine optimal medication doses based on patient age, weight, and renal/hepatic function.
- Function in collaboration with the hematology/oncology and inpatient pharmacy team, with both inpatient and outpatient responsibilities.
- Facilitate safe and timely preparation of investigational and standard of care medications for clinical trials.
- Create and maintain a pharmacy binder which contains a study summary, protocol, dispensing procedures, completes samples of required paperwork, dispensing/inventory forms, and any other relevant materials for each investigational trial.
- Attend hematology/oncology rounds, and provided pharmacy services in the interdisciplinary practice at Carle Foundation Hospital.
- Collaborate with hematology/oncology providers to enhance patient-centered care.
- Provide safe and effective drug therapy management to patients
- Review, monitor and manage adverse effects
- Monitor the care plan for regimen-specific adherence and toxicities to patient’s oral chemotherapy regimen
- Oversaw and directed the preparation of chemotherapy
- Assisted with transitions of care, supported clinical trial protocols, staff development, and precepted hematology/oncology students and residents
- Participate in treatment plan and therapy plan development and collaborate with appropriate committees for implementation
- Develop medication-related policies, procedures and protocols, and promoted safety initiatives related to the oncology patient
- Participate in pharmacokinetics dosing service activities, hospital committees and other duties as assigned
- Order Processing: Interpret physician medication orders and distribute the appropriate medications accurately and on time.
- Clinical Screening: Screen all orders for potential interactions with other medications, diet orders, disease states, and allergies.
- Controlled Substances: Maintain an accurate and complete accounting of all controlled substances.
- Technician Supervision: Supervise pharmacy technicians in the safe preparation and dispensing of medications and parenterals.
- Staff Development: Assist in the training and orientation of all new pharmacy employees.
- Committee Representation: Act as the pharmacy representative on assigned committees and work groups.
- Research & Evaluation: Assist in focused research studies and drug usage evaluations as assigned.
Compensation and Benefits The compensation range for this position is $52.06per hour - $89.54per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
