Position Highlights: Hospital Based Outpatient setting. EMR integrated with all Central Maine Healthcare providers Practice hours, Weekdays, 7 to 5:30. Individualized patient care. Provides medically prescribed physical therapy services to evaluate, treat, and restore function while improving mobility, reducing pain, and preventing disability. Delivers patient-centered care in accordance with hospital policies, regulatory requirements, and evidence-based practices. • Perform patient evaluations and physical therapy assessments • Develop and implement individualized treatment plans • Provide therapeutic interventions for rehabilitation and functional improvement • Monitor patient progress and adjust treatment plans as needed • Educate patients and families on treatment plans and recovery goals • Document evaluations, treatments, and patient outcomes accurately • Collaborate with physicians and interdisciplinary healthcare teams • Maintain compliance with regulatory, licensing, and patient safety standards Qualifications:
