Maintain and/or improve residents' ability to manage everyday physical, mental, and psychosocial needs
Identify individual social and emotional needs
Develop and maintain individualized care plans
Participate in planning for admissions, community re-entry, or transfers to other facilities
Collaborate in interdisciplinary care plan meetings to address residents' social and emotional needs
REQUIREMENTS:
Bachelor of Social Work (BSW)
or
Master of Social Work (MSW) required
Degree xhyhwjd must be in Social Work (candidates with Human Services or Psychology degrees will not be considered) Long-term care or subacute experience required
