Occupational Therapist

CenterWell Home Health seeks a compassionate Occupational Therapist to deliver high-quality, in-home rehabilitative care. In this role, you will perform functional assessments, create individualized treatment plans, and help adult and geriatric patients maximize independence with daily activities. You will collaborate with an interdisciplinary team, educate families on adaptive strategies and home safety, and document outcomes in our EMR.
