Collaboration , we want to hear from you. What You'll Do: Help identify and support residents' social and emotional needs
Participate in care planning with our interdisciplinary team
Build strong relationships with residents and their families
Provide guidance, resources, and education
Lead or assist with resident engagement activities
Maintain required documentation and training
What Youll Need: Bachelors degree in Social Work preferred
Current LSW (Licensed Social Worker) required
Experience with older adults is a plus
