Requirements: Board Certified / Board Eligible in
FM, IM, EM, or OM
REQUIRED
Active
Ohio license
REQUIRED (due to quick start)
DEA & BLS REQUIRED
Residency in Family Medicine, Internal Medicine, Emergency Medicine, or Occupational Medicine
