Overview:
Sentara Health is seeking to hire a qualified individual to join our team as a
Registered Nurse Case Manager.
Overview:
The Direct Care Registered Nurse - Home Health and Hospice Provides high-quality nursing care to patients in their homes or hospice settings, ensuring comfort, dignity, and emotional support during end-of-life care.
Anticipated Status/Shift:
Full Time (40 hours/week)
Day Shift
Position Location:
Sentara Hospice Services - Chesapeake, VA
Pay Range: $33.61- $56/hour
We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits.
Education Associate's Degree in Nursing (Required)
Certification/Licensure Valid RN License in state of practice (VA) (Required)
BLS (Required within 90 days of hire)
Experience 1 year of nursing experience (Required)
Previous Home Care/Hospice experience (Preferred)
Prior case management experience (Preferred)
