NFI North, Inc. is seeking Health Coordinator to work with our medical team at our 16-bed Transitional Housing Program in Concord, NH. This program is a residential treatment and day program with a progressive reputation of providing comprehensive clinical, vocational services and life skills for adults living with mental illness that are seeking independent living. Many of our participants get apartments and go to college with the support of our amazing staff team.
Responsibilities: Provide direct care to participants, oversee medication administration while on shift, complete medical documentation and manage external provider visits and programmatic medical supplies. Coordinate with an NFI registered nurse to provide medication administration oversight as needed. Transporting participants to medical appointments and scheduling future appointments. The schedule for this position is Monday Friday 8AM-4PM.
Qualifications: Medication trained and certified for at least two years to include having 2 or more years of experience passing medications in a residential or healthcare setting.
