Provide initial intake screenings and wellness assessments to determine client eligibility and coordinate with health and wellness programming. Prepare and keep data, files, reports, drafts, and accurate reporting of intake/referral/list activities. Collect fiscal data to assist with the completion of monthly, quarterly, and annual reports. Manages daily assignments and ensures deadlines are met and work is completed correctly. Provide information and community referrals to ineligible individuals. Assist the team in conducting service referrals for internal and external resources. Develop and maintain a comprehensive referral network with community and Wellness Center partners to connect clients with essential services. Support planning and coordination of the programs in the facility and its activities. Responsible for conducting routine psycho-social-emotional needs assessments, completing Person-Centered Recovery Plans, assisting responders and their families with accessing community resources, providing rehabilitative services, and initiating crisis intervention when needed. Promote wellness, community integration, independent living within the community and improved quality of life. A high level of detailed electronic documentation is required and adherence to company and state guidelines is expected Obtain, store, and track all client data and coordinate wellness assessments in Salesforce. Project management or event planning experience is highly desirable. Other administrative duties as assigned. Education: Undergraduate degree in business administration, Non-Profit management, public health or related field from an accredited college or university, or relevant work experience can be used in place of degree. Experience: Required 3 years of experience relevant to the position and working with vulnerable populations. Preferred 2 years of experience in Wellness.
