High School Diploma or equivalent required Valid New York State HHA Certification required (or equivalent approved training/exam) OR Certified Nursing Assistants (CNAs) willing to complete HHA conversion testing are encouraged to apply Prior experience in assisted living, home care, or long-term care preferred Medication assistance experience preferred (training provided) Basic computer skills for EMR documentation
Key Responsibilities: Resident Care & Support
Provide hands-on assistance with activities of daily living (ADLs), including bathing, dressing, toileting, grooming, and mobility Promote resident independence, dignity, and choice in all interactions Conduct routine overnight rounds to ensure resident safety, comfort, and well-being Respond promptly to call lights and resident needs
Medication Assistance
Assist residents with self-administration of medications in accordance with ALP guidelines and facility policies Follow all medication assistance protocols, including proper documentation and reporting Immediately report any medication concerns, refusals, or discrepancies to licensed nursing staff Do not interpret physician orders or independently administer medications outside of scope
Monitoring & Reporting
Observe, document, and report changes in resident condition, behavior, or needs to the Charge Nurse/DPS Accurately complete documentation in the electronic medical record (EMR) in a timely manner Report incidents, safety concerns, or unusual findings per policy
Care Coordination & Teamwork
Communicate effectively with nursing staff, leadership, and interdisciplinary team members Support a safe and respectful environment for residents, families, and coworkers Participate in shift-to-shift communication to ensure continuity of care
Environment & Support Services
Provide light housekeeping and resident support services as needed Assist with meal-related needs, hydration, and nourishment monitoring (as applicable overnight) Follow all procedures related to admissions, discharges, and transfers
